Where a student intends to participate in an accredited elite sports program or elite arts program in which they will be absent from school for a significant period of time, this form is to be completed by the students' parent or caregiver.
This application is to be completed by the parent or caregiver of the student/s fo leave of 5 days or more for the purpose of travel within Australia and/or overseas. This leave application must be either emailed or sent to the school office at least 14 days prior to leave.
If your child is taking leave from school (during the school term), a letter outling the leave dates and reason, must be either emailed or sent to the school office at least 14 days prior to leave.
To be completed by prescribing doctor and handed in to the school together with Form 1.
